Replacement Degree Order Form


PHCC will issue replacement degrees, diplomas, certificate and career student certificates previously awarded to students for a $10 fee per credential, provided the graduate does not have an outstanding debt to the college.
Prior to requesting a replacement degree:
⇛ Contact the Office of Admissions and Records at 276-656-0346 to confirm that you were awarded a credential; then
⇛ Contact/visit the Business Office- 276-656-0210/West Hall (room 155) and pay the applicable fee; then
⇛ Complete this form and submit.

Our staff will receive the form once submitted. If you are paying by phone, please ask the business office to forward a copy of your receipt to the Records office. If you are paying in person, you can bring your receipt in person.

Please note that credentials are not mailed from the college but from a third party vendor called Parchment. You will receive an email to claim your digital credential prior to delivery of your hard copy via US mail. Please ensure that you list your current email and mailing address below to ensure delivery. Mailings can take up to three weeks after processing.
