The purpose of this form is to determine if courses previously taken at another VCCS college
can be used to meet program requirements at PHCC.
If you attended another college in the Virginia Community College System, and would like for the
Office of Admissions to evaluate those courses, you must complete the information below and submit
this form to Jessica Carter, Coordinator of Admissions and Records.
